Share your feedback through our Annual Survey

If you’ve used one of our services, we’d really value hearing from you.

Our Annual Survey helps us understand what has been helpful, what difference our support makes, and how we can improve. Your feedback also helps us understand who we are reaching across Aotearoa and whether we are meeting the needs of different communities.

The survey is anonymous, takes only a few minutes, and you can skip any question you don’t want to answer.

Your responses help us:

  • improve the support we offer

  • plan for the future

  • understand the needs of our community

  • show the value of our services to funders and supporters

If you’ve used our Helpline, we may ask a few additional questions about your experience and what changed for you after reaching out.

Wellbeing Survey Prize Draw – Terms & Conditions
1. Eligibility

Entry into the prize draw is open to individuals who complete the annual wellbeing survey during the promotion period.

2. Promotion period

The survey (and associated prize draw) opens on 15 July and closes at 11:59pm on 15 August. Entries received after this time will not be eligible.

3. Entry method

Each completed survey submission equals one entry into the prize draw.

4. The prize

There are three (3) wellbeing packs to be won. Each pack includes:

  • A physical Wellbeing Workbook

  • A Keep Card

  • A selection of stickers

Prizes are not transferable, refundable, or exchangeable for cash.

5. Draw process

The prize draw will take place on 18 August.
All eligible entries will be compiled into a spreadsheet. Winners will be selected a random number generator.

Winners will be contacted via email using the contact details provided in the survey.

Winners must reply within two (2) weeks of being notified, providing a valid postal address for delivery.
If a winner does not respond within this timeframe, the prize will be forfeited and may be redrawn.

Prizes will be sent via standard NZ Post in a standard envelope. The organiser is not responsible for delays or loss in transit.

No correspondence will be entered into regarding the outcome of the draw or the awarding of prizes.

6. General

By entering the draw, participants agree to these Terms & Conditions. The organiser reserves the right to amend or cancel the promotion if necessary.
